- Adjuster information
- Nurse Case Manager Information
- Date of Injury
- Claim Number
- Employer information
- Office visit notes are sent to the
Adjuster and Nurse Case Manager within 2 work days
of the office visit.
- Work status is sent to adjuster and nurse Case Manager
the same day as the office visit.
- Surgical and testing requests are submitted to Workers’
Compensation in a timely manner.
- On site Workers’ Compensation Pharmacy services.
- On site MRI and EMG services if authorized.
- Independent Medical Examinations
